FICS Volunteer Forms
The Florida Institute for Community Studies (FICS) needs mentors, tutors and volunteers for multiple positions and special events Monday - Saturday throughout the year. Volunteer Orientation is held at 6:30 p.m. on the 2nd and 4th Wednesday of every month and only takes 90 minutes, so come attend and get started!!! View the volunteer application before your orientation and be sure you have all of the necessary information with you. If you have access to a printer, print and complete the application now.

Parental Consent & Medical Waiver (250KB pdf)
Minors need a signed parental consent form to be able to volunteer, and all volunteers need to complete medical waivers and provide emergency contacts. You can view, print, and complete these forms now.
Multimedia Consent (79KB pdf)
Volunteers at FICS may be photographed or videotaped by FICS staff or news reporters for promotional purposes only. You will need to sign a multimedia consent form to give us permission to use your image.
